Patient Home Sleep Testing Questionnaire, have patients complete and fax with copy of front and back of insurance cards and drivers license to 703-444-4308.

Home Sleep Testing (HST) Reference Guide

Initialize Device:
1. From Internet Explorer go to: www.SleepHeart.com/atHome and click on the "Sign In" tab in upper right.
2. Enter Username: firstname.lastname and Password.
3. At Welcome Screen - plug in HST device via USB cable and wait for it to be recognized.
4. Microsoft's auto-play will open the HST drive or prompt to scan (VISTA). Cancel or close window.
5. Click on New Study from the menu on the left.
6. Enter required fields marked by a red asterisk.
7. Click on Initialize Study in bottom right hand corner. If the button is grayed out click on the Find Device button in the top section.
8. Click on OK when prompted that Initialization was completed successfully. Verify hours of battery charge available in top section on left "Battery Status".
9. Have patient complete the SleepHeart@Home Medical HST Questionnaire.
10. Unplug device from computer, attach disposables, clean device and send home with patient.

Upload Study:
1. From Internet Explorer go to: www.SleepHeart.com/atHome and click on the "Sign In" tab in upper right. * VISTA users should execute Internet Explorer by right-clicking icon and selecting "Run As Administrator". *
2. Enter Username: firstname.lastname and Password.
3. At Welcome Screen - plug in HST device via USB cable and wait for it to be recognized.
4. Microsoft's auto-play will open the HST drive or prompt to scan (VISTA). Cancel or close window.
5. Click on Process Study from the menu on the left.
6. Enter required fields marked by a red asterisk. For "Interpretation Required", select "yes".
7. Click on Upload Study in bottom right hand corner.
8. Complete SleepHeart Medical HST Questionnaire pop-up screen and select Save.
9. Unplug device from USB cable when prompted that process study was completed successfully.

Reading Reports:
1. From Internet Explorer go to: www.SleepHeart.com.com/atHome and click on the "Sign In" tab in upper right.
2. Enter Username: firstname.lastname and Password.
3. Click on Reports, then Study Reports from the menu on the left.
4. Filter by patient name or date range and then click on Search to pull up reports.
